Responsibilities

• Provide Personal Care Services to clients per care plan.
• Assist with ADLs and IADLs and other health-related tasks.

• Assist with light housekeeping, meal prep, companionship, errands, and transportation arrangements.
• Identify safety hazards in the client’s home and report concerns to supervisor.
• Provide clients with social and emotional support.
• Maintain confidentiality and respect for all clients.
• Maintain required training and education.

Position Duties Include:

• Provide Personal Care Services to clients in accordance with the care plan.
• Provide assistance including, but not limited to, Activities of Daily Living (ADLs), Instrumental Activities of Daily Living (IADLs), and other health related tasks.
• Document cares provided to client as instructed.
• Assist with routine household chores of light housekeeping, meal preparation, companionship, errands/shopping, and transportation arraignments.
• Recognize safety hazards in client's home environment or habits of daily living.
• Report all safety concerns to supervisor in a timely manner.
• Provide clients with social and emotional support.
• Maintain strict confidentiality and treat all individuals with respect.
• Maintain training and educational requirements.

Qualifications

CFSS Support Worker Certification, High School Diploma or GED, and industry knowledge strongly preferred.
