Job summary

Circle of Life is seeking a CFSS Support Worker for the Winnebago, MN area. This part-time role (12-15 hours/week) focuses on providing compassionate in-home care to clients, following individualized care plans and ensuring safety and dignity in daily activities.

Responsibilities include personal care, ADLs/IADLs, companionship, light housekeeping, meal prep, and transportation coordination, with strict confidentiality and ongoing training requirements.

Qualifications

• CFSS Support Worker Certification and a High School Diploma/GED are strongly preferred.
• Experience or knowledge in home care or related industry is advantageous.
• Ability to recognize and address safety concerns in home care settings.

Responsibilities

• Provide Personal Care Services to clients in accordance with the care plan.
• Assist with ADLs, IADLs, and other health-related tasks.
• Document cares provided to client as instructed.
• Assist with light housekeeping, meal preparation, companionship, errands/shopping, and transportation arrangements.
• Recognize safety hazards in client's home environment or habits of daily living.
• Report all safety concerns to supervisor in a timely manner.
• Provide clients with social and emotional support.
• Maintain confidentiality and treat all individuals with respect.
• Maintain training and educational requirements.

Position Duties Include:

• Provide Personal Care Services to clients in accordance with the care plan.
