Job summary

Circle of Life in Granite Falls, MN is seeking a CFSS Support Worker for a part-time role to assist clients at home. You will provide personal care, support with daily living activities, and companionship following care plans to help clients stay independent in their homes.

Qualifications

• CFSS Support Worker Certification or equivalent preferred.
• High school diploma or GED required.

Responsibilities

• Provide Personal Care Services to clients in accordance with the care plan.
• Assist with ADLs, IADLs, and other health-related tasks.
• Document cares provided to client as instructed.
• Assist with light housekeeping, meal preparation, companionship, errands/shopping, and transportation arrangements.
• Recognize safety hazards in the client’s home environment or daily living habits.
• Report safety concerns to supervisor promptly.
• Provide clients with social and emotional support.
• Maintain confidentiality and treat all individuals with respect.
• Maintain training and educational requirements.
