Qualifications

• Master’s-level fully licensed clinician (LCSW/LCMHC/LPA/LMFT) or RN.
• Three years of care management or coordination experience with child welfare.
• CCM certification required.
• Ability to gain and keep access to Hospital/Data or EMR systems.
• Valid driver’s license with current auto liability insurance.

Responsibilities

• Oversee assigned Care Managers; ensure adherence to CFSP care management model.
• Ensure care plans meet policy and quality standards; provide guidance for trauma informed care.
• Coordinate with multidisciplinary teams including primary care, behavioral health, and specialists.
• Assist with implementation of new workflows and programs to support CFSP goals.
• Maintain patient privacy by following CCNC Privacy and Security policies.
• Provide coverage for vacations or vacancies as needed.
