Job summary

Qualifications

• Master-level licensure (LCSW/LCMHC/LPA/LMFT) or RN license.
• Three years of care management experience with child welfare.
• CCM certification required.
• Supervising care managers for in-reach and transition.

Responsibilities

• Manage and oversee assigned Care Managers.
• Ensure staff are trained and compliant with goals and policies.
• Review Care Plans for quality and guide care managers.
• Coordinate with multidisciplinary care teams and stakeholders.
• Provide coverage during vacations or vacancies.
• Support implementation of new workflows and programs.
• Adhere to privacy and security policies.
• Disseminate communication and lead team meetings.
• Oversee scheduling, time sheets, and administrative duties.
