Job summary

Enhabit Home Health & Hospice is seeking a licensed therapy assistant to deliver intermittent therapy services to patients in their homes. You will educate patients and families, coordinate with the health care team, and ensure care aligns with the physician’s orders and the established plan of care (POC).

Requirements include state licensure, CPR certification, a valid driver license, and reliable transportation.

Qualifications

• Graduate of approved school of clinical education.
• Licensed to practice in the state where you currently practice.
• Minimum one year experience as a licensed therapy assistant.
• Medicare home health or hospice experience preferred.

Responsibilities

• Administer therapy services to patients requiring intermittent professional services.
• Teach the patient, family, and other health care team members.
• Perform services in accordance with physician orders and the plan of care (POC).
