Job summary

Flagstar Rehab is seeking a Certified Occupational Therapy Assistant (COTA) for a Skilled Nursing Facility. You will collaborate with an OT team, follow Plans of Care, and deliver quality therapy to patients with complex conditions.

Position requires NYS OTA license, EMR proficiency, and willingness to work in a dynamic rehab setting. Independent contractor options available; W-2 paid weekly with comprehensive benefits.

Qualifications

• Current NYS OTA license and registration required.
• EMR competence and documentation in SNF settings.
• State medical documentation (TB, MMRV, vaccines) required.

Responsibilities

• Collaborate daily with OT supervisor and follow Plan of Care.
• Provide quality therapy services to SNF patients (various diagnoses).
• Monitor and document patient progress using EMR software.
• Proficiency with Rehab Optima and Point Click a plus.

Hourly Pay Rate: $34-$36 Hourly (W-2), $38-$40 Hourly (1099)

Occupational Therapy Assistant Job Duties:

• Collaborate daily with OT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in a SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.
• Proficiency in Rehab Optima + Point Click a plus!

Occupational Therapy Assistant Requirements:

• Current NYS OTA License and Registration
• EMR Competence
• State Required Medical Documentations for work in a Skilled Nursing Facility (Physical, TB Test, MMRV Vaccine Record or Titers, Covid Vaccine Documentation)
• Valid Proof of Identity and Work Authorization
