Flagstar Rehab in New York seeks a Certified Occupational Therapy Assistant (COTA) to join our skilled nursing facility team. You will work with the OT supervisor, follow the established Plan of Care, and deliver quality therapy to patients with diverse diagnoses. Documentation of progress uses EMR and inpatient care standards are followed. We support Direct Employment or Independent Contractor arrangements.

Qualifications

• Current NYS OTA license and registration.
• EMR competence.
• State required medical documentation for SNF work (physical, TB test, MMRV vaccine/titers, Covid).
• Proof of identity and work authorization.

Responsibilities

• Collaborate daily with OT Supervisor and follow Plan of Care.
• Provide therapy to SNF patients with varied diagnoses.
• Monitor and document patient progress in EMR.

Hourly Pay Rate: $34-$36 Hourly (W-2), $38-$40 Hourly (1099)

Occupational Therapy Assistant Job Duties:

• Collaborate daily with OT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in a SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.
