Job summary

Flagstar Rehab is seeking a Certified Occupational Therapy Assistant (COTA) to join our Rehab Department in a skilled nursing facility. You will work with a diverse, multi-disciplinary team to provide therapy and educate patients and families to improve function and cognition.

We offer the choice of Direct Employment or Independent Contractor, with weekly pay and comprehensive benefits. NYS OTA license required; bilingual English/Russian preferred; schedules are Flexible (FT/PT/Per Diem).

Qualifications

• Current NYS OTA license and registration must be active.
• EMR software proficiency is required.
• Medical documentation for SNF work must be up to date.

Responsibilities

• Collaborate daily with OT Supervisor and follow Plan of Care.
• Provide quality therapy services in a SNF setting for patients with mobility and cognitive needs.
• Monitor and document patient progress using EMR software.

Hourly Pay Rate: $34-$36 Hourly (W-2), $38-$40 Hourly (1099)

Occupational Therapy Assistant Job Duties:

• Collaborate daily with OT Supervisor and follow established Plan of Care for all patients.

• Provide quality therapy services to individuals in a SNF setting, who require care related to TBI, CVA, Stroke, Parkinson's, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.

• Monitor and document patient progress with the use of innovative EMR software.

Occupational Therapy Assistant Requirements:

• Current NYS OTA License and Registration

• EMR Competence

• State Required Medical Documentations for work in a Skilled Nursing Facility (Physical, TB Test, MMRV Vaccine Record or Titers, Covid Vaccine Documentation)

• Valid Proof of Identity and Work Authorization

• Malpractice/Liability Insurance (if applicable)

• BILINGUAL English/Russian Preferred.
