Job summary

Access Home Health in Jacksonville, FL is seeking a Certified Occupational Therapy Assistant (COTA) to deliver individualized care in patients' homes. Full-time and PRN positions offer flexible scheduling, one-on-one patient interaction, and strong clinical support from a responsive leadership team.

Responsibilities include implementing treatment plans, guiding exercises and activities, documenting progress, and educating families.

Qualifications

• Active Florida COTA License.
• CPR Certification.
• Home health experience preferred but not required.

Responsibilities

• Assist in the implementation of individualized treatment plans developed by the Occupational Therapist.
• Provide therapeutic interventions to patients, including exercises, activities, and modalities.
• Monitor and document patient progress and adjust treatment plans as necessary.
• Educate patients and their families on therapeutic techniques and home exercise programs.
• Collaborate with the Occupational Therapist and other healthcare professionals to ensure comprehensive care for patients.
• Maintain accurate and up-to-date patient records.

Compensation

• Salary Range:$55,000-75,000/year

• Routine Visit: 1.0 Unit
