Lone Tree Post Acute in Antioch, CA is seeking a Certified Occupational Therapy Assistant (COTA) to join our in-house rehabilitation team in a 99-bed skilled nursing facility. You will provide skilled OT treatments, help residents regain independence, and collaborate with the interdisciplinary team.

• Provide skilled occupational therapy treatments under the supervision of the Occupational Therapist.

• Educate residents, families, and caregivers on therapy techniques, adaptive equipment, and home safety.
• Assist with discharge planning to support safe transitions home.
• Maintain accurate, timely clinical documentation in compliance with regulatory standards.
• Foster a positive and encouraging therapy environment that motivates residents throughout their rehabilitation journey.
