Job summary

Premier Home Health Providers Inc. is seeking a per diem Occupational Therapist to support home-bound patients across Napa, CA. The OT will assess environments, design treatment plans, and educate families, ensuring safe, independent living at home.

Role emphasizes teamwork, compassionate care, and flexible scheduling with travel to patient homes. Per diem rate is $130 per visit and includes mileage reimbursement.

Qualifications

• Graduate of an OT curriculum accredited by AOTA/CAAHEA.
• Registered with NBCOT.
• Active California OT license required.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and care team.
• Teach and retrain patients in daily living activities affected by illness or injury.
• Recommend and instruct on adaptive equipment, splints, and orthotics.
• Maintain accurate clinical records and report changes in patient condition.
• Coordinate with care team for seamless patient-centered care.
• Educate family members and caregivers on treatment approach.
• Supervise OTAs and oversee home health aide care when OT is the sole skilled service.
• Support discharge planning as patients transition out of home health care.
• Perform other duties as required.

Responsibilities

• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices
• Maintain accurate clinical records and promptly report changes in patient condition
• Coordinate with the broader care team to ensure seamless, patient-centered care
• Educate family members and caregivers on the patient’s treatment approach
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition out of home health care
• Perform other duties as required for patient services

• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
• Registered with the National Registration Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes
• Strong communication and organizational skills
