Job summary

Premier Home Health Providers Inc. is seeking an Occupational Therapist to support patients in their homes on a per diem basis. You will evaluate functional abilities, develop treatment plans, and teach daily living activities to help patients regain independence.

Requirements include CA licensure, NBCOT eligibility, CPR, and a valid driver’s license with reliable vehicle for travel between patient homes. Flexible schedule and potential for full-time conversion based on performance.

Qualifications

• OT degree from accredited program.
• California OT license required.
• NBCOT registration or eligibility.
• Minimum 2 years clinical OT experience.
• CPR certification with AHA.
• Valid driver’s license and reliable insured vehicle.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in daily living skills affected by illness or injury.
• Recommend adaptive equipment, splints, and prosthetic/orthotic devices.
• Maintain accurate clinical records and report changes in patient condition.
• Coordinate with the care team to ensure seamless, patient-centered care.
• Educate family members and caregivers on the treatment approach.
• Supervise OT Assistants and oversee home health aide care when OT is the sole skilled service.
• Support discharge planning as patients transition out of home health care.

Please note that this role is per diem with a rate of $110 - $130 per visit.

• Coordinate with the broader care team to ensure seamless, patient-centered care
• Educate family members and caregivers on the patient’s treatment approach
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition out of home health care
• Perform other duties as required for patient services

• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
• Registered with the National Registration Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes
• Strong communication and organizational skills
