Job summary

Premier Home Health Providers Inc. is seeking a per diem Occupational Therapist to support patients in their homes across New York. You will evaluate functional abilities, develop individualized treatment plans, and educate families to promote independence and safe daily living.

Requirements include an OT degree, NBCOT registration, active California license, CPR/AHA, and a willingness to travel to patient homes. Flexible schedule and mileage reimbursement are offered.

Qualifications

• Graduate OT curriculum accredited by CAAHEA and AOTA.
• Registered NBCOT and active California OT license.
• CPR certification with AHA and driving eligible.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns
• Develop personalized treatment plans and communicate them to physicians and the care team
• Teach and retrain patients in daily living skills affected by illness or injury
• Recommend and instruct patients on adaptive equipment and devices
• Maintain accurate clinical records and report changes in patient condition
• Coordinate with the care team for patient-centered home health care
• Educate family members and caregivers on treatment approach
• Supervise OT Assistants and oversee home health aide care when OT is sole skilled service
• Support discharge planning for home health care
• Perform other duties as required

Please note that this role is per diem with a rate of $110 - $130 per visit.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns
• Develop personalized treatment plans and communicate them to physicians and the care team
• Teach and retrain patients in daily living skills affected by illness or injury
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices
• Maintain accurate clinical records and promptly report changes in patient condition
• Coordinate with the broader care team to ensure seamless, patient-centered care
• Educate family members and caregivers on the patient’s treatment approach
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition out of home health care
• Perform other duties as required for patient services

• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
• Registered with the National Registration Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes
• Strong communication and organizational skills
