Job summary

Premier Home Health Providers Inc. seeks an Occupational Therapist to help patients regain independence and safely navigate daily living at home. You will assess functional abilities and design personalized therapy plans to promote recovery in the patient’s own home environment.

The role is per diem with a rate of $110-$130 per visit, requiring CA licensure, 2+ years of OT experience, and ability to travel to patient homes; a supportive, family-oriented team culture is offered.

Qualifications

• Graduated OT from an accredited program and eligible for CA licensure.
• Registered with the National Registration Examination for OT (NBCOT) or equivalent.
• Minimum 2 years of clinical OT experience in home health or related setting.
• CPR certification (AHA) current.
• Valid California driver’s license with reliable insured vehicle for patient visits.
• Excellent communication and organizational skills.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify safety needs.
• Develop personalized OT treatment plans and coordinate with physicians and care team.
• Teach and retrain patients in daily living activities affected by illness or injury.
• Recommend adaptive equipment, splints, and orthotic devices as needed.
• Maintain accurate clinical records and promptly report changes in patient condition.
• Coordinate with the broader care team for seamless, patient-centered care.
• Educate family members and caregivers on treatment approaches.
• Supervise OT Assistants and oversee home health aides when OT is the sole skilled service.
• Support discharge planning as patients transition home.
• Perform other duties as required for patient services.

• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association


• Registered with the National Registration Examination of the American Occupational Therapy Association


• Active California Occupational Therapy license


• Minimum 2 years of clinical OT experience


• Current CPR certification with AHA


• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes


• Strong communication and organizational skills
