Job summary

Rocky Mountain Home Health and Hospice - Utah is seeking a Certified Nursing Assistant to provide direct resident care in a long-term care setting under supervision. You will assist with activities of daily living, meals, mobility, and documentation, while following care plans and facility policies.

The role requires a current CNA license (state in good standing preferred), CPR/BLS and a Food Handler Permit, with prior LTC experience beneficial.

Qualifications

• Certified Nursing Assistant in good standing with the state (preferred, not required).
• High school diploma or equivalent.
• Current CPR/BLS certification
• Current Food Handler Permit
• Must have no disciplinary action in effect against certification.
• At least one year of experience as a certified nursing assistant in long-term care (preferred, not required).

Responsibilities

• Assist residents with activities of daily living according to care plans and policies.
• Assist with lifting, turning, positioning, and transferring residents.
• Coordinate dining room services, meal delivery, feeding assistance, and document intake.
• Record weights and report changes to nursing staff.
• Support skin integrity and report any concerns to nurses.
• Assist with toileting programs and daily care documentation.
• Communicate effectively with residents, families, and staff.
• Adhere to infection control policies and safety procedures.
• Attend care-plan meetings and participate in quality improvement activities.
