Job summary

Carbondale Rehabilitation and Healthcare Center is seeking a Certified Nursing Assistant (CNA) for the second shift (2PM–10PM) on a full-time basis. You will provide hands-on, compassionate care to residents and work closely with a supportive team in a busy skilled nursing environment.

Qualifications

• Active CNA certification in Pennsylvania.

Responsibilities

• Provide hands-on, person-centered care that helps residents feel safe, comfortable, and valued.
• Answer call lights promptly and respond to residents’ needs.
• Assist with activities of daily living (bathing, dressing, grooming, toileting).
• Support safe mobility, transfers, and ambulation.
• Turn and reposition residents to promote comfort and protect skin integrity.
• Assist with meals and encourage proper nutrition and hydration.
• Observe residents and promptly report changes to nursing staff.
• Accurately document vital signs, intake/output, and other required information.
• Maintain a clean, safe, and respectful environment.
• Offer emotional support and companionship.

What you’ll do as a CNA on second shift
You’ll deliver hands-on, person-centered care that helps residents feel safe, comfortable, and valued throughout the afternoon and evening.

• Answer call lights promptly and respond to residents’ personal needs

• Assist with activities of daily living (bathing, dressing, grooming, toileting)

• Support safe mobility, transfers, and ambulation

• Turn and reposition residents to promote comfort and protect skin integrity

• Assist with meals and encourage proper nutrition and hydration

• Closely observe residents and promptly report any changes to nursing staff

• Accurately measure and document vital signs, intake/output, and other required information

• Maintain a clean, safe, and respectful environment

• Offer genuine emotional support and companionship during quieter moments

What we’re looking for

• Active Certified Nursing Assistant (CNA) certification in Pennsylvania

• Strong communication and teamwork skills
