Job summary

Carbondale Rehabilitation and Healthcare Center, a skilled nursing facility in Pennsylvania, is seeking a Certified Nursing Assistant for the second shift (2PM-10PM). You will provide compassionate overnight care, respond to call lights, assist with activities of daily living, and help residents stay safe and comfortable.

New graduates are welcome. Competitive pay from $18.00 to $23.00 per hour, paid breaks, PTO, health insurance, and 401(k).

Responsibilities

• Provide compassionate overnight care to residents.
• Respond promptly to call lights and requests.
• Assist with activities of daily living (bathing, dressing, grooming, toileting).
• Support mobility and safe transfers.
• Turn and reposition residents for comfort and skin health.
• Assist with meals and monitor nutrition/hydration.
• Observe residents and report changes to nursing staff.
• Record vital signs and intake/output accurately.
• Maintain a clean, safe environment.
• Provide emotional support and companionship during quiet hours.

What You’ll Do

As a CNA on 2nd Shift, you will:

• Deliver excellent, compassionate care to our residents throughout the night

• Answer call lights promptly and respond to personal needs with patience and respect

• Assist with activities of daily living (bathing, dressing, grooming, toileting)

• Support mobility, transfers, and ambulation safely

• Turn and reposition residents to promote comfort and prevent skin issues

• Assist with meals and ensure proper nutrition and hydration

• Observe residents closely and promptly report any changes in condition to nursing staff

• Accurately measure and document vital signs, intake/output, and other required information

• Maintain a clean, safe, and respectful environment

• Provide emotional support and genuine companionship during quieter hours

What You Bring

• Active Certified Nursing Assistant (CNA) certification in Pennsylvania

• Strong communication and teamwork skills

• A compassionate, patient-centered approach to care

• Previous skilled nursing or long-term care experience preferred—but not required. New graduates are welcome and encouraged to apply!
