Company: Pleasant Hill Post Acute
Location: Pleasant Hill, CA
Job Type: Part-Time
Shift: All Shifts
Salary: $22.00 – $25.00 per hour (Depending on Experience)
At Pleasant Hill Post Acute, we are dedicated to providing outstanding, compassionate care to our residents. We pride ourselves on creating an atmosphere of warmth, personal interest, and positive energy. If you are a dedicated Certified Nursing Assistant (CNA) looking to make a meaningful difference in a supportive and professional environment, we want you on our team!
The primary purpose of your job position is to provide each of your assigned residents with routine daily nursing care and services in accordance with the resident’s assessment and care plan, and as may be directed by your supervisors.
Assist residents with daily functions including dental/mouth care, bathing, dressing, undressing, and grooming (combing hair). Assist with lifting, turning, moving, positioning, and safely transporting residents into and out of beds, chairs, bathtubs, wheelchairs, and lifts. Perform restorative and rehabilitative procedures as instructed. Ensure residents remain dry and comfortable by promptly changing gowns, clothing, and linens when wet or soiled. Deliver after-meal care (removing trays, cleaning residents' hands, face, and clothing) and help residents identify food arrangements on their trays. Routinely check on residents—especially those unable to call for help—to ensure all personal care needs are met promptly and according to their wishes.
Participate in and receive the nursing report upon reporting for duty. Measure and accurately record temperatures, pulses, and respirations (TPRs), as well as resident weights. Accurately record residents’ food and fluid intake; report any changes in eating habits to the Nurse Supervisor/Charge Nurse. Observe disoriented and comatose residents, recording and reporting data as instructed. Safely perform special treatments and nursing care procedures that you have been trained to handle. Actively observe and report the presence of pressure areas, skin breakdowns, or injuries of an unknown source (including skin tears) to prevent bedsores.
Ensure resident rooms are perfectly prepared for arrivals (beds made, name tags up, admission kits ready). Warmly greet new residents, escort them to their rooms, and introduce them to roommates and staff. Help residents safely pack, inventory, mark, and store their personal possessions during admissions, room transfers, or discharges. Maintain clean, fresh water pitchers within easy reach of residents on every shift.
Properly identify residents using wristbands or photo card files before administering treatments or serving meals. Record all entries on flow sheets, notes, and charts in an informative, descriptive manner using only authorized facility abbreviations. Promptly report all changes in a resident’s condition, complaints, grievances, or instances of a resident leaving/missing from the facility to the Nurse Supervisor. Immediately report all observed accidents, incidents, occupational exposures (blood, hazardous chemicals), or defective equipment. Strictly maintain the confidentiality of all resident protected health information (PHI) and honor a resident's refusal of treatment, reporting it immediately to your supervisor.
Certification: Must possess a current, valid Certified Nursing Assistant (CNA) certification.
