Job summary

The Care Team Hospice is seeking a compassionate PART-TIME Certified Nursing Assistant (CNA) to join our team. You will provide in-home and facility-based personal care, support families, and help maintain patient dignity and comfort on a daily basis.

As part of an interdisciplinary team, you will document patient care, monitor changes in condition, and ensure a safe, respectful environment for hospice patients. A valid CNA certificate and dependable transportation are required.

Qualifications

• Active CNA certification required.
• Hospice/home health or long-term care experience preferred.
• Reliable, compassionate caregiver with strong communication skills.

Responsibilities

• Provide personal care, bathing, grooming, hygiene assistance, and comfort measures per individualized plan of care.
• Observe, document, and report changes in patient condition to supervising nurse.
• Support patients and families with dignity, respect, and comfort to enhance quality of life.
• Assist with mobility and activities of daily living while maintaining a safe environment.
• Complete accurate documentation in the electronic medical record (Homecare Homebase).
