Job summary

Sunplex Subacute Center is seeking a dedicated Certified Nursing Assistant to provide compassionate direct patient care on the 2pm–10pm shift. You will assist residents with daily living activities, monitor changes, and document care as directed.

Qualifications

• Certified Nursing Assistant certification in good standing is required.

Responsibilities

• Conduct direct care rounds on the unit to monitor activity and assist with activities of daily living.
• Maintain positive relationships with residents and patients.
• Notify the Charge Nurse of any change in resident condition.
• Obtain vital signs, weights, and heights as instructed by the charge nurse.
• Complete all assigned duties and required documentation.
• Coordinate admissions, transfers, and discharges in the Transitional Care Unit.
• Maintain a safe and clean work area and follow infection prevention practices.
• Perform other duties as assigned.

Full time | Shift hours: 2pm to 10pm

Certified Nursing Assistant ( CNA ) duties include but are not limited to:

• Conduct regular direct care rounds on assigned unit to monitor activity and ensure residents needs are met including assisting with activities of daily living
• Maintain positive relationships with the residents/ Patients
• Notify Charge Nurse of any resident/ patient change in condition
• Obtain vital signs, weights, and heights as instructed by charge nurse
• Complete all assigned duties and documentation as required
• Coordinate and prepare for new admissions, transfers, discharge of residents in the Transitional Care Unit
• Maintain safe and clean work area
• Ensure infection prevention and control practices
• Other duties as assigned
