Woburn Rehabilitation and Nursing Center - Woburn, MA

Job Summary: We are seeking compassionate, motivated, and energetic Certified Nursing Assistants (CNAs) to join our outstanding nursing team at Woburn Rehabilitation and Nursing Center, a dynamic facility with a commitment to providing exceptional care to our residents.

Responsibilities:

• Provide supportive services to nurses and other staff as needed
• Assist residents with activities of daily living, including bathing, grooming, toileting, dressing, and feeding
• Assist with lifting, turning, moving, positioning, and transporting residents safely
• Coordinate dining room services: meal setup, tray delivery, feeding assistance, cleanup, and documentation
• Deliver nutritional supplements and ensure proper intake; document accordingly
• Weigh residents and report changes to nurse and supervisor
• Track and report skin condition, including pressure areas, skin breakdown, or skin tears
• Assay with toileting program activities
• Complete daily flow sheets and document progress for each resident
• Keep nurses and supervisors informed of factors affecting care delivery (e.g., resident refusal, reassignment)
• Report changes in resident condition promptly

Qualifications:

• Current, unencumbered Massachusetts CNA certification
• High school diploma or equivalent preferred
• Strong communication skills
• Ability to provide compassionate, professional, and patient-centered care
• Prior long-term care experience preferred but not required
