Job summary

Moments Hospice is seeking a compassionate CNA to support patient care on the road. You will perform daily living activities, review care plans, assist with transfers, and document in the EMR under the supervision of a Case Manager RN.

You must hold an active CNA license with 12 months caregiving experience. A valid driver's license and auto insurance are required, CPR is preferred, and you will benefit from a competitive salary and a full benefits package including a company car and cell phone.

Qualifications

Certified Nursing Assistant (CNA) license is required.
12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice required.
Current CPR certification preferred.

Responsibilities

Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care).
Review patient care plans.
Assist with lifting and transferring patients.
Assist with range of motion, positioning, exercise, and ambulation.
Assist with dining and nutrition.
Maintain electronic medical records.
Work under the direction of a Case Manager RN.
