Job summary

Moments Hospice is seeking a compassionate CNA to provide personal care and support under the supervision of a Case Manager RN. You will perform daily living activities, review care plans, and maintain electronic records while traveling on the road.

Qualifications

• CNA license required.
• 12 months caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice required.
• Current CPR certification preferred.
• Excellent customer service skills.
• Strong communication (written, verbal, active listening).
• Problem-solving and time management skills.
• Ability to work in an electronic medical record system.
• Ability to lift 50 lbs and good body mechanics.
• Strong understanding of professional boundaries.
• Valid driver's license required.
• Valid auto insurance required.

Responsibilities

• Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.).
• Review patient care plans.
• Assist with lifting and transferring patients.
• Assist with range of motion, positioning, exercise, and ambulation.
• Assist with dining and nutrition.
• Maintain electronic medical records.
• Work under the direction of a Case Manager RN.
