Job summary

Moments Hospice in Wisconsin is seeking a compassionate CNA to provide daily personal care, support, and comfort to patients in their final days. You will work under an RN Case Manager, review care plans, and help with mobility, grooming, meals, and daily activities.

The role emphasizes teamwork, strong communication, and adherence to patient dignity, with a competitive hourly wage of 18–23 USD, a company car, health insurance, and PTO.

Qualifications

• CNA license required, 12 months caregiving experience in relevant settings.
• CPR certification preferred.
• Strong communication, time management, and problem-solving abilities.
• Able to work in an electronic medical record system.

Responsibilities

• Provide personal care (bathing, dressing, grooming, toileting, skin care).
• Review patient care plans with RN Case Manager.
• Assist with lifting, transferring, ROM, positioning, and ambulation.
• Assist with dining and nutrition and maintain EMR documentation.
• Follow guidance from RN Case Manager and ensure patient comfort.

Qualifications:

• Certified Nursing Assistant (CNA) license (required)
• 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required)
• Current CPR certification (preferred)
• Excellent customer service skills
• Strong communication (written, verbal, and active listening)
• Problem-solving skills
• Time management skills
• Ability to work in an electronic medical record system
• Ability to lift 50 lbs.
• Good understanding of body mechanics
• Strong understanding of professional boundaries
• Valid driver's license (required)
• Valid Auto Insurance (required)

Responsibilities:
