Job summary

Moments Hospice is seeking a Certified Nursing Assistant (CNA) to provide compassionate in-home care on the road, coordinating with the RN Case Manager to ensure patient comfort and dignity. A strong emphasis on personal care and documentation will be required, with on-call support and a competitive benefits package.

Qualifications

• CNA license is required.
• 12 months of caregiving experience in home care, assisted living/memory care, skilled nursing, hospital or hospice.
• Current CPR certification preferred.
• Excellent customer service skills.
• Strong communication (written, verbal, and active listening).
• Problem-solving, time management, and ability to lift 50 lbs.

Responsibilities

• Perform personal care activities (bathing, dressing, grooming, toileting, skin care).
• Review patient care plans.
• Assist with lifting and transferring patients.
• Assist with range of motion, positioning, exercise, and ambulation.
• Assist with dining and nutrition.
• Maintain electronic medical records.
• Work under the direction of a Case Manager RN.

Qualifications
