Job summary

On With Life in Polk City is seeking a compassionate Certified Nursing Assistant (CNA) to join our rehabilitative care team. You will provide direct care under supervision and help residents with daily activities to promote independence.

Responsibilities

Skilled Long‑Term Care (Polk City, Iowa)

• Location: Polk City, Iowa
• Hours: 6:00 PM‑6:30 AM or 6:00 AM‑6:30 PM (3 shifts /week) rotating holidays & every 3rd weekend required
• Staffing ratios: Nurses 1:10 all shifts, CNAs 1:5 on days & 1:10 nights

Post‑Acute Care (Ankeny, Iowa)

• Part‑Time Schedule: 6:00 AM‑2:30 PM Weekend Package
• Part‑Time Schedule: 6:00 AM‑6:30 PM (2 shifts /week) rotating holidays & every 3rd weekend required
• Full‑Time Schedule: 2:00 PM‑10:30 PM (4‑5 shifts /week) rotating holidays & every 3rd weekend required
• Full‑Time Schedule: 6:00 PM‑6:30 AM (3 shifts /week) rotating holidays & every 3rd weekend required

IMPORTANT: Please note that our orientation process (typically up to 1 month) may require working days or shifts outside of what will be your permanent schedule.

• Assist with transfers, including use of mechanical lifts.
• Perform range‑of‑motion exercises as directed by therapy staff.

• Help persons served achieve their highest quality of life.

Requirements for Certified Nursing Assistant (CNA)

• Current Iowa CNA certification
• Polk City: Must be 18 years of age (required to operate mechanical lifts)
• Ankeny: Must be 16 years of age
• Maintain CPR certification (training available if not certified)
• Maintain mandatory reporter certifications (training available if not certified)
