Job summary

Moments Hospice is seeking a compassionate CNA to provide direct personal care to patients in a home health setting. You will work under the supervision of an RN Case Manager, performing activities of daily living and assisting with mobility while maintaining patient comfort and dignity.

Qualifications

CNA license required.
12 months caregiving experience required.
Current CPR certification preferred.
Excellent customer service skills.
Strong written and verbal communication.

Experience with electronic medical records is a plus.
Ability to lift 50 lbs and understand body mechanics.
Maintain professional boundaries and patient confidentiality.
Valid driver's license required and valid auto insurance.

Responsibilities

Perform personal care activities (bathing, dressing, grooming, toileting, skincare).
Review patient care plans and assist with transfers.
Assist with ROM, positioning, exercise, and ambulation.
Assist with dining and nutrition.
Maintain electronic medical records under RN Case Manager guidance.

Qualifications:

Certified Nursing Assistant (CNA) license (required)
12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required)
Current CPR certification (preferred)
