Job summary

Evergreen Post Acute in Portland, OR is hiring a full-time on-site Certified Nursing Assistant (CNA) to provide compassionate resident care, assist with activities of daily living, monitor health, and support therapy and nursing interventions.

Role Description This is a full-time, on-site Certified Nursing Assistant (CNA) role based in Portland, OR. The CNA provides direct resident care including assistance with activities of daily living such as bathing, dressing, grooming, toileting, feeding, and mobility support. Daily responsibilities include monitoring and recording vital signs, observing and reporting changes in resident condition, and supporting nurses and therapists with treatments and rehabilitation activities. The role involves helping residents move safely, maintaining clean and comfortable living spaces, and following infection control and safety protocols. CNAs also offer emotional support, promote engagement in social activities, and communicate effectively with residents, families, and the interdisciplinary care team.

Qualifications

• Current Certified Nursing Assistant (CNA) certification in the state of Oregon and compliance with all applicable licensing requirements.
• Demonstrated skills in resident care and activities of daily living, including safe transfer techniques, personal hygiene support, and mobility assistance.
• Ability to accurately measure and document vital signs, observe changes in resident status, and report concerns promptly to nursing staff.
• Knowledge of basic infection control, safety procedures, and proper handling of equipment in a skilled nursing and rehabilitation setting.
• Strong communication and interpersonal skills to interact respectfully with residents, families, and colleagues from diverse backgrounds.
• Capacity to work effectively as part of an interdisciplinary team, following care plans and supporting therapy and nursing interventions.
• Physical ability to meet job demands, including lifting, positioning, and extended periods of standing or walking, with or without reasonable accommodation.
• Prior experience in long-term care, post-acute, or rehabilitation facilities is preferred but not required; willingness to learn and participate in ongoing training.
