Job summary

Bend Transitional Care is seeking a full-time Certified Nursing Assistant (CNA I) to join our nursing team in Bend, Oregon. We provide compassionate transitional and post-hospital care to help residents regain strength, independence, and confidence as they work toward returning home or transitioning to their next stage of care.

You’ll provide daily living assistance, support mobility and transfers, monitor vital signs, assist with meals, document changes, and collaborate with nurses to deliver

Qualifications

  • High school diploma or equivalent required.
  • Valid CPR certificate required.
  • Six months experience in a long term-care environment (preferred but not required).

Responsibilities

  • Providing assistance with activities of daily living, including bathing, dressing, grooming, and personal hygiene.
  • Assisting residents with mobility, transfers, and range-of-motion exercises.
  • Taking and documenting vital signs, food and fluid intake, and output as directed.
  • Assisting residents during meals and supporting their nutritional needs.
  • Observing residents and promptly communicating changes in condition to the nursing team.
  • Maintaining accurate and timely documentation.
  • Building positive, respectful relationships with residents, families, and team members.
  • Working collaboratively with the nursing team to provide exceptional resident care.

We provide compassionate transitional and post-hospital care to help our residents regain their strength, independence, and confidence as they work toward returning home or transitioning to their next stage of care. Requirements

Education:
  • High school diploma or equivalent required.
Licenses/Certification:
  • Valid certification as a CNA in Oregon
  • Valid CPR certificate
Experience:
