Evergreen Post Acute is seeking a Certified Nursing Assistant to provide routine daily nursing care to residents in accordance with care plans. Wage range from $23.63 to $28.63 per hour based on experience.

The role emphasizes patient safety, accurate documentation, and teamwork with nursing staff. A CNA license and CPR are preferred; high school diploma/10th grade education is required; location Portland, Oregon.

Formación

• Must possess a CNA license per state laws.
• CPR certification preferred.

Responsabilidades

• Provide routine daily nursing care and services in accordance with the resident’s assessment and care plan.
• Identify residents before administering treatments and ensure accurate records.
• Report changes in residents’ condition to the Nurse Supervisor/Charge Nurse promptly.
• Record entries on flow sheets, notes, and charts in an informative, descriptive manner.
• Maintain confidentiality of resident care information and report any disclosures.
