Job summary

Home Sweet Home Health Agency is seeking a compassionate Certified Nursing Assistant to join our Home Health team serving the East and South East Valley, AZ (Mesa, Chandler, Gilbert, and surrounding communities). This role offers structured M-F hours, in-home patient care, and competitive hourly compensation.

You will work one-on-one with patients, assist with ADLs, and support the care team to promote safety and independence in the comfort of patients’ homes.

Qualifications

• Active Arizona CNA certification is required.
• Experience in Home Health, hospice, caregiver, or senior care preferred.
• Must be compassionate, dependable and patient-focused.
• Strong communication and teamwork abilities are essential.
• Valid driver’s license, reliable transportation, and current auto insurance.

Responsibilities

• Provide personal care (bathing, grooming, dressing, toileting, mobility).
• Assist with activities of daily living promoting dignity and independence.
• Support safe transfers, ambulation, and light rehab as directed by clinical team.
• Observe and report changes in patient condition to supervising nurse.
• Provide companionship and emotional support to patients and families.
• Educate caregivers on safe care techniques and follow care plans.

What You’ll Do

As a Home Health Certified Nursing Assistant (CNA), you will:

• Provide personal care, including bathing, grooming, dressing, toileting, and mobility assistance
