Job summary

Home Sweet Home is seeking a compassionate Certified Nursing Assistant to join our Home Health team in the East and South East Valley, AZ. The role focuses on one-on-one patient care in the home, with flexible territories and meaningful work delivering senior care in a community setting.

Responsibilities include ADL support, safety transfers, companionship, and education of caregivers, with competitive hourly pay and supportive clinical leadership.

Qualifications

• Active Arizona CNA certification required.
• Home Health, hospice, caregiver, or senior care experience preferred.
• Driver's license, reliable transportation, and current auto insurance.

Responsibilities

• Provide personal care, including bathing, grooming, dressing, toileting, and mobility assistance.
• Support patients with activities of daily living while promoting dignity and independence.
• Assist with safe transfers, ambulation, and light rehabilitation as directed by the clinical team.
• Observe and report changes in patient condition to the supervising nurse.
• Provide companionship and educate caregivers on safe care techniques.
• Follow individualized care plans and agency policies to ensure quality patient care.

What You'll Do

As a Home Health Certified Nursing Assistant (CNA), you will:
