Job summary

Home Sweet Home Health in Arizona is hiring a Certified Nursing Assistant to provide in-home care in Mesa, Chandler, Gilbert and surrounding communities. Schedule Monday–Friday, 8:00 AM–5:00 PM; compensation $24–$26 per hour.

As a CNA on our Home Health team, you’ll deliver compassionate one-on-one care, assist with ADLs, monitor patient status, and educate families while following individualized care plans. Join a supportive, community-focused employer.

Qualifications

• Active Arizona CNA certification required.
• Home Health, hospice, caregiver, or senior care experience preferred.
• Valid driver's license and reliable transportation.
• Strong communication and teamwork skills.

Responsibilities

• Provide personal care, including bathing, grooming, dressing, toileting, and mobility assistance.
• Support patients with activities of daily living (ADLs) while promoting dignity and independence.
• Assist with safe transfers, ambulation, and light rehabilitation support as directed by the clinical team.
• Observe and report changes in patient condition to the supervising nurse.
• Provide companionship and emotional support to patients and families.
• Educate caregivers on safe care techniques.
• Follow individualized care plans and agency policies to ensure quality patient care.
