Job summary

Home Sweet Home Health is seeking a compassionate Certified Nursing Assistant (CNA) to join our home health team serving the East and South East Valley communities in Arizona. The role offers Mon-Fri, 8am-5pm hours and pay of $24–$26 per hour with meaningful in-home patient care.

Responsibilities include personal care and ADL support, safe transfers, companionship, and reporting changes to the supervising nurse.

Qualifications

• Active Arizona CNA certification required.
• Home Health, hospice, caregiver, or senior care experience preferred.
• Compassionate, dependable, and patient-focused.
• Strong communication and teamwork skills.
• Valid driver's license, reliable transportation, and current auto insurance.

Responsibilities

• Provide personal care, including bathing, grooming, dressing, toileting, and mobility assistance
• Support patients with activities of daily living (ADLs) while promoting dignity and independence
• Assist with safe transfers, ambulation, and light rehabilitation support as directed by the clinical team
• Observe and report changes in patient condition to the supervising nurse
• Provide companionship and emotional support to patients and families
• Educate caregivers on safe care techniques
• Follow individualized care plans and agency policies to ensure quality patient care
