Job summary

Home Sweet Home Health is seeking a compassionate Certified Nursing Assistant to join our in-home care team in the East and South East Valley communities of Mesa, Chandler, and Gilbert.

This CNA role emphasizes one-on-one patient care, flexible territories, and collaboration with a supportive clinical leadership to deliver quality senior care, community health support, and meaningful work in the home setting.

Qualifications

• Active Arizona CNA certification is required.
• Experience in Home Health or senior care preferred.
• Compassionate, dependable and patient-focused.
• Strong communication and teamwork skills.

Responsibilities

• Provide personal care, including bathing, grooming, dressing, toileting, and mobility assistance.
• Support patients with activities of daily living (ADLs) while promoting dignity and independence.
• Assist with safe transfers, ambulation, and light rehabilitation support as directed by the clinical team.
• Observe and report changes in patient condition to the supervising nurse.
• Provide companionship and emotional support to patients and families.
• Educate caregivers on safe care techniques.
• Follow individualized care plans and agency policies to ensure quality patient care.

What You'll Do
