Job summary

Home Sweet Home Health is seeking a compassionate Certified Nursing Assistant to join our Home Health team serving the East and South East Valley communities, including Mesa, Chandler, and Gilbert. This full-time role offers a Monday–Friday, 8:00–5:00 schedule with one-on-one in-home patient care.

You will assist with ADLs, mobility, and observe changes in condition, reporting to the supervising nurse. We provide supportive leadership, meaningful work in the local community, and opportunities

Qualifications

• Active Arizona CNA certification is required.
• Home health, hospice, caregiver, or senior care experience is preferred.
• Compassionate, dependable, and patient-focused.

Responsibilities

• Provide in-home personal care (ADLs: bathing, grooming, dressing) and mobility assistance.
• Support daily living activities and comfort, promoting dignity and independence.
• Observe and report changes in patient condition to the supervising nurse.
• Provide companionship and educate caregivers on safe care techniques.

What You'll Do

• Provide personal care, including bathing, grooming, dressing, toileting, and mobility assistance
• Support patients with activities of daily living (ADLs) while promoting dignity and independence
• Assist with safe transfers, ambulation, and light rehabilitation support as directed by the clinical team
• Observe and report changes in patient condition to the supervising nurse
• Provide companionship and emotional support to patients and families
• Educate caregivers on safe care techniques
• Follow individualized care plans and agency policies to ensure quality patient care

Qualifications

• Active Arizona Certified Nursing Assistant (CNA) certification required
• Home Health, hospice, caregiver, or senior care experience preferred

• Strong communication and teamwork skills
• Valid driver's license, reliable transportation, and current auto insurance
