Job summary

Suncrest Hospice is seeking a compassionate Certified Nursing Assistant (CNA) to join our Mesa, AZ team. You will provide patient-centered hospice care under nursing leadership, ensuring comfort and dignity per individualized plans.

As part of a dedicated team, you will collaborate with nurses, social workers, and chaplains, adhere to policies, and travel as needed with reliable transportation. CHAP-certified care is a cornerstone of our mission.

Qualifications

• Fulfills AZ Fingerprint Clearance Card requirement.
• CNA certification in the state of practice (AZ).
• Compassionate and committed to improving patients' quality of life.
• Ability to collaborate with Nurses, Social Workers, Chaplains, Volunteers.

Responsibilities

• Provide compassionate care to hospice patients per assessments and care plans.

The Certified Nursing Assistant (CNA) works under the direction of the nursing leadership team to provide compassionate care to assigned hospice patients in accordance with their assessments and individualized care plans. The CNA Collaborates with the hospice care team to meet patient needs, promote comfort and dignity, and report changes in condition. The CNA follows all Company policies, procedures, safety guidelines, and infection‑control practices.

Qualifications

• AZ Fingerprint Clearance Card
• Compassionate and committed to improving patients’ quality of life
• Ability to work collaboratively with Nurses, Social Workers, Chaplains, Volunteers, and Aide Leadership
• Comfortable working one‑on‑one with patients while being supported by a dedicated care team
• Able to work independently and manage daily responsibilities
• Willingness to travel with reliable transportation, a valid driver’s license, and current auto insurance
• Current CNA certification in the state(s) of practice
• Eagerness to pursue additional certifications, education, and career growth opportunities
