Job summary

Suncrest Hospice is seeking a compassionate Hospice Certified Nursing Assistant (CNA) to support patients under the Nursing Leadership Team. You will provide care per individualized plans, collaborate with nurses, social workers, chaplains, volunteers and aides, and help maintain comfort and dignity for patients in their homes or facilities.

The role requires CNA certification, willingness to travel, and the ability to work independently within a dedicated care team.

Qualifications

• Current CNA certification in the state(s) of practice.
• Willingness to travel with reliable transportation.
• Comfortable working one-on-one with patients.
• Able to work independently and manage daily responsibilities.

Responsibilities

• Provide compassionate care to hospice patients per individualized care plans.
• Collaborate with the care team to meet patient needs and report changes in condition.
• Follow company policies, procedures, safety guidelines and infection-control practices.

The Hospice Certified Nursing Assistant (CNA) works under the direction of the Nursing Leadership Team to provide compassionate care to assigned hospice patients in accordance with their assessments and individualized care plans. The CNA collaborates with the hospice care team to meet patient needs, promote comfort and dignity, and report changes in condition. The CNA follows all Company policies, procedures, safety guidelines, and infection-control practices.

Qualifications

• Compassionate and committed to improving patients’ quality of life
• Ability to work collaboratively with Nurses, Social Workers, Chaplains, Volunteers, and Aide Leadership
• Comfortable working one-on-one with patients while being supported by a dedicated care team
• Able to work independently and manage daily responsibilities
• Willingness to travel with reliable transportation, a valid driver’s license, and current auto insurance
