Heritage-Woods-of-Chicago is seeking a dedicated CNA to provide individualized care in a warm, resident-centered environment in Chicago. You will support residents with daily activities, follow care plans, and collaborate with the nursing team to maintain high-quality care.

Qualifications

• High School diploma or GED required.
• Current certification from an accredited Certified Nursing Assistant (CNA) program.
• Excellent communication and interpersonal skills.

Responsibilities

• Deliver exceptional and individualized care per each resident’s care plan.
• Assist with Activities of Daily Living (ADLs) while promoting independence and dignity.
• Report changes in resident health status to the clinical team.
• Connect with residents and family members in a compassionate and impactful manner.
• Engage in continuous learning through seminars, staff meetings, and other learning experiences.
• Comply with confidentiality and privacy of resident information.
• Perform any other duties as assigned.
