Job summary

Heritage Woods of South Elgin is seeking a Certified Nursing Assistant to deliver individualized care per care plans and assist with daily living activities while maintaining residents’ dignity. You will report health changes to the clinical team and engage with residents and families in a compassionate manner.

Qualifications

• High School graduate or GED equivalent required.
• Current CNA certification from an accredited CNA program.
• Exceptional communication and interpersonal skills.
• Must demonstrate the ability to serve with love, compassion, and dignity.

Responsibilities

• Deliver exceptional and individualized care as determined by each resident’s care plan.
• Provide assistance with Activities of Daily Living (ADLs) while promoting independence and dignity.
• Report changes in resident health status to the clinical team.
• Connect with residents and family members in a compassionate and impactful manner.
• Engage in continuous learning and development through seminars, staff meetings, and other learning experiences.
• Comply with confidentiality and privacy of resident information.
• Perform any other duties as assigned.
