Job summary

Elara Caring is seeking a Certified Nurse Aide to provide in-home care across Hartford, Vernon, Granby and surrounding areas. The PRN role involves assisting with daily living activities, vital signs, and mobility support while following each patient’s care plan.

Current CT CNA/HHA certification is required. You must be able to lift up to 50 pounds and have reliable transportation to travel between client homes. Competitive pay and benefits are offered.

Responsibilities

• Provide hands-on personal care under RN supervision.
• Assist patients with bathing, dressing, grooming, toileting, and daily activities.
• Take and record vital signs: temp, pulse, respirations, BP.
• Support safe walking, transfers, and use of mobility equipment.
• Follow patient care plans and report changes to the care team.
• Maintain clean, safe home environment and protect patient privacy.

As a Certified Nurse Aide, you’ll contribute to our success in the following ways:

• Provide hands-on personal care under the direction of a Registered Nurse.
• Assist patients with bathing, dressing, grooming, toileting, and other daily activities.
• Take and record vital signs, including temperature, pulse, respirations, and blood pressure.
• Help patients safely walk, transfer, and use mobility equipment.
• Follow each patient's individualized care plan.
• Monitor and report changes in a patient's condition to the care team.
• Help maintain a clean, safe, and comfortable home environment.
• Remind patients to take medications as directed.
• Respond appropriately to emergency situations and follow safety and infection control guidelines.
• Maintain accurate documentation, protect patient privacy, and participate in required training.

What is Required?

• Current State certificate as a Home Health Aide certification or Certified Nursing Aide certification.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

Actual pay will be determined based on factors such as education, experience, skills, and internal equity. This role may also be eligible for additional compensation, which could include performance-based bonuses, commissions, on-call pay, or other incentive earnings. These components are not guaranteed and depend on individual and/or team performance.
