Job summary

CareOne Livingston is hiring Certified Home Health Aides (CHHA) to support residents with daily living activities in a warm assisted living setting. Full-time, part-time, and per diem opportunities across multiple shifts are available.

Responsibilities include ADL assistance, hygiene, mobility, meal support, and reporting changes to nursing staff. The role requires a state CHHA license, HS diploma, and willingness to travel; drivers license and transportation preferred.

Qualifications

• HS diploma or equivalent required.
• State Certified CHHA license required.
• Hospice/home health experience preferred, not required.

Responsibilities

• Assist with ADLs: personal hygiene, grooming, mouth, skin care, nails.
• Assist with toileting, positioning, and transfers; monitor bed-bound residents.
• Support prescribed exercises and ROM activities.
• Light housekeeping of resident's room and linen changes.
• Assist with meals and feeding.
• Take vitals as directed by Hospice Nurse.
• Report changes to Hospice Nurse and document appropriately.
• Support patient in taking medications per regulatory rules.

CareOne at Livingston (Assisted Living)

Salary Range: $16-$25 per Hour

Certified Home Health Aide (CHHA) Responsibilities

• Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails.
• Provides assistance in toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient.
• Assists with the prescribed exercise and ROM activities which the resident/patient and Hospice aide have been taught by the Hospice Nurse.
• Assists with light housekeeping of individual's personal unit or room, including changing of bed linen, if needed and patient laundry.
• Feeds and assists with meals.
• Obtains temperature, pulse and respirations as supervised by the Hospice Nurse.
• Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information on the Home Health Aide Certification of House form, including the date and time of the report and the person receiving the information.
• Assists or reminds the patient to take medication, in compliance with regulations of appropriate regulatory and accrediting bodies.

Position Requirements

• High School Diploma or equivalent
• Must have a State Certified CHHA license
• Experience in hospice or home health, preferred
• No experience necessary but 1 year, preferred
• Must be willing to travel
• Must have a valid driver's license and reliable transportation
