Job summary

Ascend Hospice & Palliative Care NJ in Central New Jersey is hiring a Certified Home Health Aide (CHHA) to join the Central NJ Team. The CHHA will provide hands-on personal care, assist with ADLs, and support patients in their homes under Hospice Nurse supervision.

The role requires a State Certified CHHA license, willingness to travel, reliable transportation, and a HS diploma or equivalent. Experience in hospice/home health is preferred but not required.

Qualifications

• Must have a State Certified CHHA license.
• High School Diploma or equivalent.
• Experience in hospice or home health preferred.
• Must be willing to travel.
• Must have a valid driver's license and reliable transportation.

Responsibilities

• Provides or assists in ADL activities: personal hygiene, grooming, mouth, skin care and nails.
• Assists with toileting, transfers, positioning, and frequent turning of bed-bound patients.
• Assists with prescribed exercise and ROM taught by Hospice Nurse.
• Assists with light housekeeping of the patient's unit/room and laundry.
• Feeds and assists with meals.
• Obtains temperature, pulse and respirations as supervised by the Hospice Nurse.
• Reports changes in patient condition to the Hospice Nurse and documents appropriately.
• Assists or reminds the patient to take medications as regulated.

Now Hiring -Certified Home Health Aide (CHHA) - Central NJ Team

ThisCertified Home Health Aidewill be responsible for, but not limited to:

1. Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails.
2. Provides assistance in toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient.
3. Assists with the prescribed exercise and ROM activities which the resident/patient and Hospice aide have been taught by the Hospice Nurse.
4. Assists with light housekeeping of individual's personal unit or room, including changing of bed linen, if needed and patient laundry.
5. Feeds and assists with meals.
6. Obtains temperature, pulse and respirations as supervised by the Hospice Nurse.
7. Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information on the Home Health Aide Certification of House form, including the date and time of the report and the person receiving the information.
8. Assists or reminds the patient to take medication, in compliance with regulations of appropriate regulatory and accrediting bodies.

Education:

High School Diploma or equivalent

Professional Experience:

• Must have a State Certified CHHA license
• Experience in hospice or home health, preferred
• No experience necessary but 1 year, preferred
• Must be willing to travel
• Must have a valid driver's license and reliable transportation
