Job summary

Ascend Hospice & Palliative Care NJ is seeking a Certified Home Health Aide (CHHA) to provide compassionate care to hospice patients in New Jersey. The role covers essential ADL support, personal care, and mobility assistance under the supervision of the Hospice Nurse.

Responsibilities include assisting with toileting and transfers, light housekeeping, meal support, and routine vital checks. Travel to patient homes is required, and a valid CHHA license is necessary to join our team.

Qualifications

• High school diploma or equivalent required.
• State Certified CHHA license required or must be obtained prior to start.
• Hospice or home health experience preferred but not required.
• Must be willing to travel and have reliable transportation.

Responsibilities

• Provides or assists in ADL activities, personal hygiene, grooming, mouth, skin care and nails.
• Assists with toileting, positioning and transfer activities; frequent turning and positioning of bed-bound residents/patients.
• Assists with prescribed exercises and ROM as taught by Hospice Nurse.
• Assists with light housekeeping of the individual's unit/room, bed linen changes, and patient laundry.
• Feeds and assists with meals.
• Obtains temperature, pulse and respirations as supervised by the Hospice Nurse.
• Reports changes in resident/patient condition to Hospice Nurse and documents information on required reports.
• Assists or reminds patient to take medications in compliance with regulations.

ThisCertified Home Health Aidewill be responsible for, but not limited to:

• Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails.
• Provides assistance in toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient.
• Assists with the prescribed exercise and ROM activities which the resident/patient and Hospice aide have been taught by the Hospice Nurse.
• Assists with light housekeeping of individual's personal unit or room, including changing of bed linen, if needed and patient laundry.
• Feeds and assists with meals.
• Obtains temperature, pulse and respirations as supervised by the Hospice Nurse.
• Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information on the Home Health Aide Certification of House form, including the date and time of the report and the person receiving the information.
• Assists or reminds the patient to take medication, in compliance with regulations of appropriate regulatory and accrediting bodies.

Education:
