Macon Resources Inc. in Decatur, IL is seeking a qualified Case Manager/QIDP. Under the supervision of the Director of Community Day Services, you will train staff and individuals served, implement Person Centered Plans, and monitor outcomes.

The position runs Monday through Friday, 8:00 am to 4:30 pm, with a focus on empowerment and community integration. The role emphasizes direct training, coordination with IDTs, and ongoing assessment to identify needs and preferences, while ensuring

Qualifications

Bachelor's degree in human services or related field is required.
Must meet the requirement of a Qualified Intellectual Disability Professional (QIDP).

Responsibilities

Facilitates development of the PCP or implementation strategy for each individual using normalization, choice, and community inclusion principles.
Directs implementation of the PCP/Implementation Strategy as recommended by the IDT; re-evaluates objectives monthly.
Provides guidance to CDS trainers for active training in daily living, recreation, community integration, vocational skills, safety and self-advocacy.
Develops and administers staff training; communicates policies and ensures compliance; coaches managers in curriculum development for CDS.
Maintains person-centered communication with other programs to coordinate service delivery and assists with documentation of results.
