Qualifications

• Bachelor's degree in a human services–related field is required.
• One year of cumulative direct-service experience with individuals with developmental disabilities.

Responsibilities

• Assess individual needs and coordinate appropriate services and supports.
• Complete the PAS process in compliance with PAS Manual requirements.
• Conduct required visits, Discovery, and Personal Plan development.
• Advocate for individuals and families to access eligible and preferred services.
• Review and monitor medical, residential, vocational, educational, legal, and financial needs.
• Provide problem-solving consultation with provider agencies per DHS guidelines.
• Maintain accurate documentation, daily billings, and agency compliance.
• Participate in staff meetings and required trainings.
• Communicate trends or service gaps to the Program Manager.
