Trinity Services, Inc. in New Lenox, Illinois, seeks a Case Manager (formerly QIDP) to oversee person-centered planning and coordination for individuals receiving supports and services.

You will supervise and guide a Community Support Team, ensure plan implementation, and promote empowerment, community integration, and program compliance. The role emphasizes data management, advocacy, and leadership in delivering high-quality, rights-based supports.

Formación

• Bachelor’s degree in a Human Services field as defined by the Illinois Department of Human Services
• At least one year of experience supporting individuals with intellectual/developmental disabilities
• Must meet state and federal requirements for QIDP status and be approved by IDHS
• Experience working with and guiding diverse support staff and individuals with varying abilities

Responsabilidades

• Guide and lead the Community Support Team in delivering individualized supports and services aligned with each person’s desired goals and outcomes.
• Conduct Personal Outcome Measure interviews annually or as needed/requested.
• Ensure individuals' preferences and goals are accurately reflected in person-centered documents and updated as required.
• Facilitate Community Support Team meetings to celebrate accomplishments, develop and update Flourish Plans, and address care-related decisions.
• Provide direct support to people in the home and community to help them achieve their goals and outcomes.
• Promote natural support networks to assist with the development and maintenance of personal relationships.
