Qualifications

• CDCA certification is required.
• Current State of Ohio Driver's License and active insurance.
• Must pass BCI check, all Corporate Compliance checks, and employment drug screen.

Responsibilities

• Assess client needs and develop a comprehensive plan.
• Coordinate services across life domains with the clinical team.
• Ensure care management aligns with treatment plans and HIPAA rules.
• Complete billing and documentation in a timely manner.
