Lighthouse Behavioral Health Solutions (LBHS) is hiring a Case Manager (CDCA) in Lancaster, OH. The role focuses on assessing client needs and developing comprehensive care plans within the continuum of services LBHS provides, including outpatient and CPST support.

The position reports to a Case Management Supervisor. It emphasizes coordination with clinical staff, adherence to confidentiality laws, and ongoing collaboration with community resources to promote recovery.

Qualifications

• CDCA certification required.
• Current State of Ohio Driver’s License and active insurance.
• Must pass BCI check, all Corporate Compliance checks, and employment drug screen.

Responsibilities

• Coordinate and provide safe, timely, effective, equitable, culturally sensitive, and client-centered care.
• For SUD clients: handle case assignments, assess barriers to recovery across life domains, may draft service plan, review progress.
• For MH clients: handle case assignments, assess barriers to recovery, coordinate mental health treatment planning with CPST goals, collaborate with clinicians.
• Familiar with the diagnostic assessment completed for clients served, including review of diagnosis and identified needs.
• Work collaboratively with the clinical staff in development of the treatment plan.
• Adhere to Lighthouse’s protocol for care management within the continuum of services.
• Provide referral to community resources and coordinate services for continuity of care.
• Stay updated on best practices in intervention modalities.

Education

CDCA certification

Bachelor’s Degree in Social Work, Psychology, Counseling, or directly related human service field

2 years relevant experience in case management, social services, or a related field

Position: Case Manager (CDCA)

Job Summary: Provide support services and foster community by assessing the needs of their clients and developing and implementing a comprehensive plan to address those needs. This position reports to a Case Management Supervisor.

DutiesandResponsibilities:

• Coordinatesandprovidessafe,timely,effective,efficient,equitable,culturallysensitive,andclient- centered care.
• ForSUDClients:Handlescaseassignments,assessesbarrierstorecoveryinthevariouslifedomains, may draft SUD casemanagement-specific service plan, review progress in case management goals.
• ForMHClients:Handlescaseassignments,assessesbarrierstorecoveryinthevariouslifedomains, coordinatesmentalhealthtreatmentplanningincludinggoalsforCPSTservices,providesinput,and works collaboratively with the clinical counselor, psychiatrist, and/or nurse practitioner in the development of the treatment plan.
• Isfamiliarwiththediagnosticassessmentcompletedfortheclientsserved,whichincludesareviewof diagnosis, identification of current symptoms, risk, functioning,identifiedneeds in the client’s life domains shown as Z-Codes todetermineeligibility for Case Management or CPST services and the level of care needed.
• Workscollaborativelyfollowingupwithobservationswiththeclinicalcounselor,psychiatrist,and/or nurse practitioner in the development of the treatment plan.
• AdherestoLighthouse’sprotocolforappropriate“caremanagement”activitiesaspartofthecontinuum ofservices to meet each client’s defined needs and as defined in the treatment plan.
• Provides follow-up andappropriate referralsto community resources are essential. Collaboration and coordinationofservicestoensurecontinuityofcareandefficiencyofservicesacrosssystemsisvitalto early recovery.
• Provideslinkageandassistanceinaccessinginpatientormorerestrictivetreatment,asmedicallynecessary.
• Staysabreastofbestpracticesinmodalitiesofinterventiontomaintainhigh-qualitycaremanagement that is medically necessary to client specific treatment plan.
• Evaluatesdailylivingfunctioningandassistclientsincreatinggoalstoimprovedailyliving.
• Completesbillinganddocumentationaccuratelyinatimelymanner.
• Seesclientsweeklyorasclinicallyappropriate accordingto theclient’slevelofcare.
• EnsuresallcommunityactivitiescomplywithHIPAA,42CFRPart2,andMentalHealthlawsand regulations onconfidentiality of PHI.
• Keepsabreastofcommunityresourcesandmakesneededreferralstomeetclientneedsonatimelybasis.
• Coordinatesandcollaborateswithothercommunityagenciesregardingclient’streatmenttopromote positive treatmentoutcomes. Participate in community collaboration meetings.
• Advocatesandraisesawarenessonbehalfofclientsforcommunityandlocalservicestomeetclientneeds.

Required Qualifications:

• CDCA certification

Schedule : Monday - Friday 8:30am - 5:00pm

Compensation range : $20.19 - $26.60
